*** UPDATE - My parcel arrived and was a really lovely package with 15 different designs, 3 sheets of each design so overall 45 plain A4 sheets of printing paper with the plain integrated sticky labels on each, which is so ideal for sending parcels or large letters, for invoices or printing, for labelling and writing or drawing and so much more. Included were 15 demonstrative sheets showing where and how to measure including size specs of printed matter to position accurately info for the labels. Will definitely be looking at this company first in regards to ordering printing paper when I run out.
*I ticked each box for the choices, if you want to tick less or a box or two in particular, there are pics of the labels in the menu (just click on shop) they all have the corresponding number to match each box or try all 16, will edit an update on how that goes 
*the form does ask for a company name, if it helps any, a company name can be a voluntary organisation too and a sole trader rather than a heap of people, so if you do lots of projects and help others (people or animals/ birds etc) through your creativity, then it is perfectly plausible to write in a name that represents this activity, i.e 'Feathered Freedom', 'Good Deeds', 'Fab Fabrics'... etc!

You are very welcome, I used some stickers for a sensory game I created for a child I was assisting with her living skills, it was her favourite game, it was literally the stickiness of the sticker that was used, so these labels would have worked even better
